About the role
In essence we are looking for a Registered Nurse to join the team as a Community Clinician who will be responsible for ensuring our clients reliable, safe, quality care, supports and services. This is a permanent part time role, the days to be worked are flexible, except we will require this person to work Fridays.
Key functions/duties include, but not limited to:
Clinical Care Management – Assessment and review of care plans as well as overseeing day-to-day clinical evaluations and care & behaviour support planning.
Education & Team Development – providing leadership, support, mentoring, clinical supervision/direction and upskilling of care team to ensure that clients receive optimal clinical care.
Quality Improvement, Risk Management and WHS – Monitoring and management of the documentation of the clinical practice and decision making to ensure it complies with the company policy.
About you
The main requirement for this role is to be an AHPRA Registered Nurse and to have prior experience in a community setting, supporting people with disabilities and/or aged care needs, with demonstrable knowledge of working within this sector.
Other key requirements of the role:
Strong knowledge and experience of working with people with disability.
Genuine empathy with great customer service skills – you will be dealing with a highly diverse range of people in our community.
High level of understanding around the NDIS quality and safeguards.
Relationship Building – you need to have strong influencing skills and the ability to build and develop key relationships.
Access to your own vehicle – we reimburse for KMs when out seeing our clients.
