About the role

We are seeking an experienced Clinical Case Manager (Registered Nurse) to support our Aged, Disability and severe personal injury clients in Sydney. Ideally, you will have strong nursing and case management skills and enjoy working with clients in the community.

Your role will include providing nursing procedures, managing client intake, assessments and care planning, case management, training and supporting care/support workers and coordinating and monitoring all aspects of service to ensure clients' needs are met.

Occasional regional travel required.

About you

AHPRA Registration as a Registered Nurse with experience in community-based nursing (preferred)

Experience in supporting clients in aged care, NDIS, and/or insurance schemes (icare, Workers Compensation)

Passion for patient-centred care grounded in high-quality clinical outcomes.

Outstanding interpersonal, communication, and problem-solving skills.

Ability to work within multi-disciplinary teams.

Enjoy a mix of working remotely, visiting clients' homes and in-office.

Valid Australian driver's license and access to a reliable car with comprehensive insurance.

NDIS Worker Screening and Working With Children Check (or willing to obtain).

The salary package is negotiable and will include a car allowance and the opportunity to earn performance-based bonuses.
