Case Manager | Community Recovery | Redland Bay

Employment Term: Full-time, maximum term to February 2026

Location: Redland Bay

Remuneration: $100k + Super + NFP salary packaging of up to $15,900 and $2,650 for meals & entertainment

Your part in our UnitingCare team

The primary focus of the Case Manager is to deliver comprehensive case management to individuals and families referred from the Community Recovery Program for ongoing support.

The role is grounded in a trauma-informed, client-centred approach, empowering clients to strengthen their capacity to manage life’s challenges.

You will work collaboratively with clients and community stakeholders to foster inclusive, resilient community connections and support sustainable
outcomes

This position will:

Provide person centred support through a trauma informed lens to clients that have been referred through the Community Recovery program for case management.

Conduct outreach visits to see clients in the community, homes or other appropriate settings to assess needs and provide support.

Collaborate with community partners (e.g., Neighbourhood Centres) to enhance client engagement and access to services, ensuring all interactions and referrals
are accurately recorded to monitor outcomes and inform continuous improvement.

Refer clients to appropriate external supports based on assessed needs, including but not limited to mental health services, housing support, financial assistance, and specialist care.

Undertake screening, triage and risk assessment process to all email, phone and face to face referrals, with Service Leader oversight as required

What makes a difference for us?

Tertiary Qualification in a relevant field (e.g., Social Work, Psychology or Social Science) as required by relevant service contract/s.

Lesser formal qualifications with acquisition of considerable skills and extensive and diverse relevant experience to an equivalent standard.

Current drivers licence.

Positive Blue Card.

Police History Check.
