Join Our Team as a Case Manager – Support Independence and Choice at Home

About the Role:
As a Case Manager, you will provide high-quality care and support to Commonwealth Home Support Program (CHSP) consumers. Guided by the philosophy of enabling individuals to remain living at home, you will work closely with consumers, their families, carers, and advocates to identify goals that shape their personalised Care Plan.
Your role will involve applying professional judgement to design flexible and tailored solutions that meet each consumer’s unique needs, while ensuring alignment with program eligibility requirements and agreed risk parameters.

What We’re Looking For:
- Experience in the use of computer programs e.g. Microsoft applications.

- DHS National Disability Insurance Scheme (NDIS) Worker Check
- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
- Immunisation Risk for this position is - Category A
