- Salary Range: $67,538 - $92,227, per annum, plus superannuation
South West Healthcare (SWH) is looking for a passionate and committed Aboriginal Liaison Officer to join our team and make a real impact on improving health outcomes for Aboriginal and Torres Strait Islander people living with chronic and complex conditions.

As an Aboriginal Liaison Officer, you will play a vital role in helping clients navigate the healthcare system and access the services they need. You'll focus on early intervention, self-management, and culturally safe care pathways for people living with chronic illness.
This is a highly collaborative role where you will work closely with hospitals, community health, aged care services, and Aboriginal Community Controlled Health Organisations (ACCHOs) to strengthen partnerships and improve outcomes.
In this role, you will:
- Advocate for Aboriginal patients and their families across acute and emergency care settings.
- Collaborate with clinicians to ensure care is culturally appropriate, respectful, and patient-focused.
- Support clients in developing personalised, goal-driven care plans.
- Foster strong partnerships between SWH, ACCHOs, and other health service providers.
- Provide health education tailored to individual needs and encourage participation in self-management programs.
- Contribute to capacity building across SWH through education, training, and quality improvement
What you bring
We are looking for someone who is committed to supporting Aboriginal and Torres Strait Islander communities and passionate about improving health outcomes. To be successful, you'll bring:
- Relevant qualifications in Nursing, Allied Health, or Aboriginal Health
- Strong skills in assessment, care planning, and service delivery
- The ability to work both independently and as part of a collaborative team
- Excellent communication skills and a culturally sensitive approach
- An understanding of community-based supports, services, and networks
- Experience working in an acute or community health setting
- Great time management and the ability to prioritise competing demands
