About the Role
Blossom Community Care is seeking highly experienced Disability Support Workers to provide 24/7 Supported Independent Living (SIL) care for a participant in Doreen, VIC. This role requires reliability, flexibility across a rotating roster, and strong skills in SIL documentation, compliance, and note-keeping.

Key Responsibilities

Deliver high-quality, person-centred support, including personal care and daily living assistance.

Support participant goals, community engagement, and independence.

Maintain accurate SIL documentation, compliance records, and progress notes.

Uphold confidentiality, safety, and ethical standards at all times.

Interpersonal Skills

Passion and commitment to providing high-level support.

Genuine interest in participant well-being and inclusion.

Focused on goal-oriented outcomes.

Able to work effectively both independently and in a team.

Strong verbal and written English communication skills.

Comfortable using smartphones, apps, and computers.

Professional and able to liaise with various stakeholders.

Qualifications & Experience

Completed in:

Certificate III in Individual Support

Certificate IV in Disability or Aged Care

Diploma of Nursing (or equivalent)

Proven experience in disability/community care with strong SIL knowledge.

Position Requirements

ABN required – subcontractor arrangement.

Availability for a range of shifts including nights, weekends, and public holidays.

NDIS Worker Screening Check.

Current First Aid and CPR.

Working with Children Check.

Valid Australian driver’s licence and reliable, insured vehicle.

Smartphone with data.
