About the Role
As a Community Worker at Koala Life Care, you will play a vital role in supporting individuals, families, and communities by addressing barriers such as housing insecurity, substance misuse, mental health challenges, disability, and social isolation. You will work closely with hospitals, service providers, and community agencies to provide holistic, person-centred support that empowers participants to live independently and with dignity. This is a full-time position (38 hours per week)
This is a hands-on, client-facing role ideal for someone passionate about advocacy, crisis response, and complex case coordination.

Housing and Homelessness
Mental Health and Psychosocial Disability
Alcohol and Other Drugs (AOD) Support
Community Participation and Engagement
Therapeutic and Family Support
Our approach ensures tailored support that meets the unique needs of both youth and adults in these critical areas.
Core Responsibilities
Manage referrals and complete intake assessments to determine client needs
Develop individualised care plans, incorporating psychosocial and risk assessments
Coordinate a range of supports including NDIS, housing, health, and social services
Advocate on behalf of clients in meetings with external stakeholders
Monitor and evaluate client progress, adjusting plans where required
Facilitate access to emergency and long-term accommodation
Assist with hospital discharges, ensuring continuity of care and safe transition
Provide ongoing support to individuals at risk of homelessness, substance abuse, or re-hospitalisation
Conduct regular case conferences, maintain documentation, and participate in review meetings
Collaborate with internal and external professionals to deliver integrated care
Support individuals with disabilities to access community programs, education, and employment
Promote inclusive practices and empower clients to live fulfilling, independent lives
What You’ll Bring
Minimum 2 years of experience in community services, social work, disability, or case management
Strong understanding of the NDIS and its application within complex support environments
Knowledge of housing and homelessness systems, mental health services, and AOD programs
Excellent case coordination, communication, and advocacy skills
Resilience, cultural sensitivity, and a trauma-informed approach
Mandatory Requirements
Degree in Community Services, Social Work, or equivalent
Membership or licence from an accredited professional body such as AASW or ACWA
NDIS Worker Screening Check, Working with Children Check, Police Clearance
First Aid & CPR Certification, Infection Control, Manual Handling
Valid Driver’s Licence, reliable vehicle, and comprehensive car insurance
Relevant NDIS training (e.g. Orientation, Mealtime Management, Effective Communication)
MAPA, RAN, or similar behavioural management training (preferred)
