About the role

We are seeking a dynamic, leader who can support the day-to-day operations of the care team and provide ongoing support, training and mentoring to Care Support Partners to enhance their performance, engagement and professional development. The successful applicant will be passionate about enhancing the quality of the participant’s experience while promoting health, wellbeing and continuity of care to enable participants to age in place. This role will work closely with, and act as the 2IC to, the Care Services Manager to bring our newly developed Care team model to life, and to lead an effective care management approach. Some travel will be required.

Key Accountabilities

Provide leadership excellence through the achievement of performance indicators set out in the organisation’s strategic plan and priorities

Lead, support, coach and mentor Care Support Partners to undertake business and service improvements

Collaborate across the organisation while championing strengths-based leadership, which is ethical, results driven and future oriented

Develop a high performing culture, committed to quality and outstanding customer service

Actively identify, monitor and manage areas of key risk and lead appropriate escalation and response where required in collaboration with the Governance and Risk management team

Support optimal service delivery by overseeing comprehensive care management, ensuring the wellbeing of participants through case work, advocacy, planning and review

Selection Criteria

1. Diploma in Community Services, Human Services, Health Sector Management, Aged Care, and/or a clinical background, along with at least 5 years of relevant industry experience.

2. Proven ability to lead, motivate and manage a diverse team to achieve organizational goals and foster a collaborative and inclusive team environment

3. Experienced in Care Management preferably within the Home and Community Care sector with advanced knowledge of person-centred care

4. Demonstrated experience in service development and leading continuous quality improvement, emphasizing practice and service excellence in accordance with the Strengthened Aged Care Quality standards

5. Strong interpersonal and communication skills, demonstrating relatability and engagement. Exhibits high emotional intelligence and a proven ability to cultivate relationships and connect effectively with both internal and external stakeholders

6. Understanding of specialized service delivery within diverse groups, particularly in relation to Aboriginal and Torres Strait Islander, CALD, Veterans, Disability, Mental Health, LGBTI+, and Homelessness

Desirable

1. Relevant tertiary level qualification i.e. Bachelor of Allied Health, Social Science – Behavioral Studies, Community Services, Human Services or Nursing along with at least 3 years of relevant industry experience
