About the role
We are looking for an experienced Physiotherapist to join our growing home care business.
You will be reporting to the Home & Community General Manager and based in our head office in Heidelberg, Victoria.
As a physiotherapist working in home care, you will play a vital role in maintaining and improving the health, mobility, and independence of older adults.
The role will have a focus on establishing an internal allied health division.
Key Responsibilities:
In Home Assessments - Conduct comprehensive assessments in the client’s home environment, identifying issues and needs.
Develop individual plans - Design physiotherapy programs tailored to each client’s goals, conditions, and needs. Key focus areas may include mobility improvement, pain management, and prevention of decline.
Intervention and Therapy - Deliver therapeutic exercises and mobility training.
Falls Prevention and Education - Implement evidence-based falls prevention programs, educating clients and caregivers on safe movement and home modifications.
Care Coordination and Collaboration - Communicate with GPs, nurses, occupational therapists, and family members.
Monitoring and Reassessment - Regularly evaluate progress and adjust treatment plans as needed. Maintain accurate and up to date documentation/clinical notes.
Assistive Technology Prescription and Client Training - Assess client needs, recommend appropriate assistive technology and equipment and provide training on safe and effective use.
Lead Generation - Identify and attract potential clients ensuring a steady flow of clients.
Essential criteria:
Bachelor’s or Master’s degree in Physiotherapy and registration with the Australian Health Practitioner Regulation Agency (AHPRA).
Hold a valid driver’s license and have access to a personal vehicle.
A minimum of 2–3 years of proven clinical experience is required. Experience in aged care and/or with NDIS participants is preferred but not mandatory.
Ability to work autonomously with minimal supervision.
Strong command of the English language, with effective written and verbal communication skills.
Strong organisational skills with the ability to manage time effectively.
