JCS is a non-profit community organisation providing culturally sensitive and confidential home support and assistance services to individuals and families in South Australia since 1994. Our core services include Home & Community Support services and Home Care Packages for frail/older people over the age of 65, through the provision of domestic support, personal care, social support, transport, meals and center based day care delivered by a mix of paid and non-paid staff.
We also provide access to counselling, advocacy, information, and referral to support and assist individuals, families, older persons, migrants, new arrivals, people with disabilities, and unpaid carers. Utilising wellness and reablement approach to service delivery, we encourage independence and continuing engagement in the community in order to enhance quality of life.

About the role:
Reporting to the General Manager, the Clinical Care Partner will be part of a team responsible for the coordination and delivery of high-quality person-centered health and wellbeing care, managing cases for consumers receiving Government funded and private services including Home Care Package services. The Clinical Care Partner is responsible for the efficient and effective administrative and coordination function for JCS home support services, which includes facilitating home based assessments, support planning and providing a coordinated approach to the provision of services and support to JCS clients. This role will ensure that services are reflective of the diverse needs of individuals.
The Clinical Care Partner will promote healthy ageing and provide opportunities for social engagement to the consumers with an aim to prevent frailty, avoid decline and optimise their ability to live better lives in their own homes.
Key Responsibilities
Respond to and coordinate enquiries, intake and referrals from individuals, the My Aged Care Gateway, Regional Assessment Service, Aged Care Assessment Team and external agencies and allocate accepted referrals for assessment and registration, based on JCS policies and procedures;
Undertake person-centered assessment, risk screening and care planning with new and existing consumers to identify their goals, needs and preferences and support them to take risks to enable them to live the best life they can.
Make appropriate referrals for complex holistic assessments
Work within a team to maintain best practice in documentation by recording, monitoring, and maintaining documentation that is compliant to legislative requirements, acts and regulations including JCS policies and procedures and the Aged Care Quality Standards.
Connect and build relationship with consumers, family, significant others, and stakeholders (including doctors and other health care professionals) with an aim to understand consumers’ individual health needs and goals and deliver safe and effective care and services for the Consumers.
Support consumers to make choices that will promote their wellbeing and meet their goals by using person centered and healthy ageing approaches that is based on best practice, tailored to their needs, and optimises their health and well-being.
Participate in the development, implementation and review of policy, procedures, processes, and protocols for the community services & programs area at JCS.
Provide input on the effectiveness and efficiency of service delivery and work with other team members to implement strategies to improve services as part of the aged care reform agenda.
Qualifications, Skills & Key Selection Criteria
Relevant tertiary qualification in Allied Health/Health or equivalent witha current AHPRA registration
Minimum 2 years experience in the community services/aged care/disability/human services sector;
An understanding of the ageing process and issues effecting people from diverse backgrounds;
Demonstrated experience coordinating referrals, undertaking general client assessments, reviews and client support plans to determine client overall support needs;
High-level critical thinking
Current National Police Clearance (NPC) or willingness to obtain;
Senior First Aid certificate or willingness to obtain;
Current SA (CAR) drivers license & access to a safe and reliable car with comprehensive insurance
