Police HomeCare, situated in Adelaide, is a registered provider for both NDIS and Aged Care (Home Care Packages). We provide a variety of services to both NDIS and Aged Care.

We are currently looking for a Care Partner to join our Aged Care Team reporting to our Head of Aged Care. Your key responsibilities will include:

Commitment to contemporary home care services and practices to meet the requirements of the Aged Care Quality Standards

Visit prospective and new care participants independently and complete the onboarding process

Maintain ongoing face to face contact with our care recipients

Provide support and be an escalation point for both care recipients and care workers and if necessary, escalate any issues to the Head of Aged Care

Monitoring the quality of our care delivery to ensure the care recipient's quality of life and independence is optimised and managing risks and duty of care

Our Ideal Candidate Possesses:

1. In-Depth My Aged Care Knowledge

An exceptional understanding of Home Care Packages and My Aged Care including core principles, legislation, rules and upcoming aged care changes including the strengthened standards

The ability to work flexibly within budget constraints

2. Effective Time Management and Documentation Skills:

Exceptional time management skills

Proficiency in using CRM systems and Microsoft Office for accurate and timely record-keeping of activities.

Strong computer literacy and an ability to quickly adapt to new software programs or tools as required

Proficiency in maintaining clear and accurate case notes and other records within our systems.

3. Strong Interpersonal Skills:

Excellent interpersonal skills enabling the building and maintaining relationships with various stakeholders. This includes internal and external parties such as our care recipient's families. These skills are integral to fostering cooperation and collaboration among all involved parties

Strong verbal and written communication skills to collaborate effectively with care recipients, their families, Guardians and various stakeholders. This proficiency ensures that care recipients receive the support they require while retaining their autonomy and control over their choices in the support they receive

Must be a team player, able to perform effectively under pressure, and capable of recognising early signs of declining health our care recipients

4. Proven Experience:

A track record of success in a similar role within Aged Care, NDIS or related fields

Prior experience in a Care Coordinator or Support Coordination role or a similar NDIS/LAC/Service Coordination/Case Management position

The ability to work autonomously while also collaborating with a small, experienced, and dedicated team in our office, contributing to a cohesive and supportive work environment

An ideal candidate meeting these criteria is expected to make a significant and positive impact on our organisation and the individuals we support.

Desirable Requirements:

Tertiary qualification in Nursing (EN)

NDIS Worker Screening Check

Current Driver's License

Unrestricted Working Rights in Australia

Evidence of up to date Covid 19 vaccinations including eligible first booster

Evidence of current influenza (flu) vaccination

Current First Aid and CPR certificate

Current Manual Handling Certification

Hours will be 8:00am – 4:30pm with a rotating roster for afterhours phone calls.
