Job Description:

We are seeking compassionate and motivated Disability Support Workers (Linkers) to join our dynamic team at Link to Life. As a Linker, you will provide 1:1 direct support to individuals with psychosocial disabilities, helping them navigate daily challenges and promoting their mental well-being. Your work will involve providing personalised care, assisting with daily activities, and encouraging social inclusion.

Key Responsibilities:

Provide high-quality, person-centred care and support to individuals with psychosocial disabilities

🩷Assist participants with daily living activities such as personal care, meal preparation, and medication management

💛Support participants in engaging with their communities and participating in social and recreational activities

💚Implement individualised care plans and support strategies tailored to mental health needs

🩵Foster a positive and supportive environment that encourages participant independence and well-being

🩷Monitor and report on participant progress and any changes in their needs

💛Collaborate with other team members and healthcare professionals to ensure comprehensive care

Qualifications:

• Certificate III or IV in Disability, Mental Health, Individual Support, or equivalent (desirable but not mandatory)
• Previous experience in disability support, mental health, or a related field (preferred)
• Strong communication and interpersonal skills
• Empathy, patience, and a genuine passion for supporting individuals with psychosocial disabilities
• Ability to work both independently and as part of a team
• Lived experience of disability and/or mental health challenges (highly valued)
• Valid driver’s license and access to reliable transportation
• Current First Aid and CPR certification (or willingness to obtain)
• Ability to obtain a NDIS Worker Screening Check and Working with Children Check
