Atticus Health is seeking a compassionate and motivated Care Coordinator (Registered Nurse) to join our team. This unique role combines hands-on nursing with care management, supporting older adults to live healthier, more meaningful lives.

About the Role

Location: Based in our Hardware Lane clinic (CBD) with outreach across the city and surrounding suburbs.

Split Role: 50% clinical nursing within the clinic, 50% care coordination in the community.

Focus: Partnering with people over 50, both in clinic and at their homes, to improve their quality of life.

Key Responsibilities

Provide nursing support, including clinical assessments and care delivery in the Hardware Lane clinic.

Visit patients in their homes, building trusted relationships and supporting them to live well and independently.

Act as a Care Manager for patients aged 50+, coordinating care plans and services tailored to individual needs.

Collaborate with GPs, allied health, and community providers to ensure seamless, person-centred care.

Support patients in navigating the health system, connecting them with relevant services and resources.

Maintain accurate records and documentation in line with professional standards.

About You

Registered Nurse (AHPRA) – Essential.

A genuine “people person” – warm, empathetic and able to connect meaningfully with people over 50.

Strong interest in holistic, person-centred care and improving life quality for older Australians.

Excellent organisational and communication skills.

Current driver’s licence and willingness to travel locally to patients’ homes.
