About this Position of role
Primary purpose of this role to provide weekly direct comprehensive and intensive case management support to young people and their families of African descent with minimum supervision. This role work closely with other case managers youth mentors and staff to support the social, cultural and mental health wellbeing of African young people and their families, particularly in the youth justice systems.
Duties and responsibilities
· Provide case management support for young people under youth justice orders, in detention and those experiencing significant challenges with their family, culture and education and at elevated risk of getting into contact with policy and youth justice system.
· Develop consistent action orientated clear goals to support the young people and their family achieve their goals with particular focus on employment, re-engage in education, substance misuse, mental health services, safe housing, responsible parenting skills, anti-social behaviour, and improve community participation.
· Work collaboratively with Youth Justice and coordinate case management activities to ensure better outcome for young people and their families including participation in case review meetings.
· Supporting families to build skills to work through conflict with young people to help them reconnect and reengage.
· Building capacity in families to minimise young people’s disengagement and offending risks.
· Supporting families to navigate systems– connecting with and understanding services available for a range of needs.
· Supporting and enabling families to connect with relevant services and supports in the community to build and maintain positive prosocial connections.
· Establish and maintain consistent professional and supportive relationships with young people and their families.
Maintenance accurate and up-to date information records including case notes, financial information case plans etc ensuring data entry other information are recorded promptly on SRS and other organisational data bases
· Actively participate in regular formal and informal supervision
Adhere to all policies and procedures including maintain privacy and confidentiality of young people and their families.
· Act as a suitable adult role model and facilitate the implementation of a ‘good practice’ model of the African Village to meet the needs of disconnected young people.
Essential criteria
· Demonstrated understanding of the challenges and needs of African young people and
families.
· Demonstrated Knowledge of the Queensland Youth Justice system and Act.
· Demonstrated strong knowledge of, and ability to address the challenges faced by African children, young people and their families.
· Experience working with African young people and their families, particularly young on the Youth Justice system.
· Demonstrated ability to develop and maintain relationship with relevant stakeholders.
· Experience planning, delivering and evaluating effective early intervention programs to support young people and their families.
· Demonstrated ability to communicate effectively and build rapport with African young people, their families and other stakeholders.
· Demonstrated ability to effectively support young people to engage in education, training, employment and strengthen positive relationship with family and community.
· Demonstrated excellent written communication skills with ability to use Microsoft office365 suit, SRS and other information technology to plan and produce written reports.

Essential qualifications
Bachelor’s degree in social work, human services or Diploma in Youth work with over 4 years’ experience
Requirements
· Blue Card (Working with Children)
· Criminal History Check
· Valid QLD Driver’s Licence
