About this Position of role

Primary purpose of this role to provide leadership to case workers, youth workers and youth mentor delivering intensive case management support young people and families of African descent who are exclusively referred to our service (VSL) by the Youth Justice service centres. This role work closely with other program team leaders to support the social, cultural and mental health wellbeing of young people and their families, particularly young people in the youth justice systems.

Duties and responsibilities

· Work closely and collaboratively with Youth Justice to assess and address the criminogenic needs of each young person referred to VSL.

· Managing all incoming referral from youth justice and allocating to relevant staff

· Respond to any emergent practical or welfare issues raised by Youth Justice within one (1) hour

· Undertake weekly case review with case workers and youth workers

· Undertaking risk assessments of the young person with each case worker and youth worker

· Assisting in preparing a Service Response Plan (SRP) for the young person

· Assisting in preparing a Service Response Plan – Transition (SRP-T) for the young person

· Support staff in coordinating and delivering agreed elements of the young person’s SRP or SRP-T

· Deliver change-oriented programs to address the young person’s risk of reoffending

· Support staff to ensure young people are provided with practical support, to focus on the young person’s broader wellbeing, including responding to emergency crisis issues

· Work with other service providers to build a support network for the team and young people, to assist with their reintegration into the community

· Support staff to provide support and mentoring to prepare families and young people prior to the young person’s return from detention

· Participate in care team meetings, case reviews, local Staying on Track governance groups and other stakeholder meetings as required by Youth Justic or as outlined in the Staying on Track Practice Manual or the Program Guidelines

· Developing tools and process to support efficient and effective case deliver

· Develop or be involved in developing risk management plan

· Manage budget closely with the Finance Team

· Providing reports, summaries and information as required to the contract manager and managing director

Essential criteria

· Experience in case management and familiarity with SRS client management system

· Proven leadership in managing staff and stakeholders

· Demonstrated understanding of the challenges and needs of young people and families.

· Demonstrated Knowledge of the Queensland Youth Justice system and Act.

· Experience planning, delivering and evaluating effective rehabilitation and intervention programs to support at-risk young people and their families.

· Demonstrated excellent written communication skills with ability to use Microsoft office365 suit, SRS and other information technology to plan and produce written reports.

Essential qualifications

Tertiary qualifications in Criminology and criminal justice, Psychology, and or Social Work or related field with Minimum 3 years relevant case management experience

Requirements

· Blue Card (Working with Children)

· Police Screening Clearance (or willingness to obtain)

· Valid QLD Driver’s License
