Northern Health are seeking applications from motivated and suitably experienced Grade 2 Physiotherapist to join the HARP/Complex Care team, based at Epping Private Hospital.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.
Services are provided through our five main campuses:
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.
About the Role:
Hospital Admission Risk Programs (HARP) provides services for people who are living with chronic diseases and complex needs, and who are at risk of hospital admission.
The program delivers an integrated, coordinated healthcare, which provides the right care, at the right time, and in the right setting. Through a care coordination principle, HARP ensures the essential partnerships between the client, their carer/family, their General Practitioner (GP), hospital staff and other community organisations are developed and maintained. Care is provided by a multi-disciplinary team of Specialist Medical, Nursing and Allied Health Professionals. Their aim is to provide community-based healthcare with the focus on preventing unplanned hospital admissions or emergency department presentations and improving the quality of life for clients in the community.
The grade 2 Physiotherapist will provide high quality, evidence-based physiotherapy to patients with chronic disease or complex health needs in a centre-based setting, in their home or residential care. Care will be provided via face-to-face therapy, telehealth and group sessions. The grade 2 Physiotherapist will be a leader in the physiotherapy department, involved in service development and contribute to our clinical education program for junior staff and students.
The focus of this position is on addressing short-term mobility goals and to optimise safety and function, prevent functional decline and to inform team decision-making regarding long-term goals and service requirements.
This position is fixed term part time (46 hours per fortnight) commencing August 2025 until June 2026 - covering maternity leave
To be successful in this role you will have:
- A background which includes significant clinical experience resulting in high level of skill in the delivery of physiotherapy services for patients with multiple comorbidities, complex needs and chronic conditions
- Experience managing patients in the community (centre based and home based, individual and group settings)
- Demonstrated capability in service innovation and development.
- Demonstrates leadership skills and initiative in developing and promoting clinical care
- Well-developed interpersonal skills
- Well-developed organisational and time management skills
- Demonstrated effective verbal and written communication skills
- Skills in negotiation and conflict resolution
