Role Overview
As a Disability Support Worker at EQWELL, you'll play a pivotal role in supporting those we care for with daily living activities and fostering a comfortable, safe, and empowering
environment that encourages personal growth, independence and well-being.

This position requires a warm, attentive, and professional individual with a strong commitment to providing compassionate care, and creatively considering how to increase quality tailored care outcomes.

Key Responsibilities

Being a supportive and friendly face to participants and other stakeholders.

Provide personalised care and support to residents living with EQWELL.

Assist participants with daily activities such as dressing, bathing, and grooming.

Engage participants in social activities and encourage participation in community events.

Monitor and report any changes in participants' physical or mental health to the appropriate staff members.

Maintain a clean and safe living environment for residents at EQWELL.

Respect and promote the independence and dignity of each individual under your care.

Meet the day-to-day operational needs of the home, such as rosters, general administration, activity schedules and duties.

Participate in ongoing training and professional development to improve caregiving skills and knowledge.

Understanding and completing mandatory reporting requirements, including shift note taking, and incident reporting.

Adhering to specific care plans, and positive behaviour support plans.

Adopting and maintaining a person-centred approach in all activity within EQWELL.

Additional Responsibilities Can Include:

Collaborating with participants, families, and teams to create and implement tailored support plans.

Building strong relationships with external service providers and agencies to enhance the support network for participants.

Advocate and be the first point of support to Support Workers within your home, ensuring accurate and timely facilitation of information between them and the rest of EQWELL.

Advocate and champion for the best long-term outcomes for the Participant, by proactively implementing therapist strategies and creating internal strategies on how best to achieve long term outcomes.

Take all reasonable steps to ensure the safety of all those in our care, your team mates, and others.

About You
To succeed in this role, you should have:

A strong values-driven attitude that guides and motivates you to leave the world better than you find it.

A close affinity with our own guiding North Star values, Kindness & Righteousness.

Demonstrated experience as an Disability Support Worker or in a related caregiving field.

A caring, compassionate, and patient nature, with a strong commitment to enhancing the quality of life for others.

Strong communication skills and the ability to work independently as well as part of a team.

Flexibility to accommodate varying needs and schedules.

Relevant certifications and/or training in Community Services, Aged Care, or Disability (or equivalent).
