About the role:

As a Community Support Worker at Providence Disability & Ageing Support Services , you will be responsible for providing direct support to individuals with disabilities, facilitating their access to community resources, and promoting their overall well-being. You will work collaboratively with clients, their families, and other service providers to create and implement personalized support plans that align with each individual's goals and aspirations.

Key Responsibilities:

1. Individual Support:
- Provide compassionate and person-centered support to clients in various settings, including their homes and community environments.
- Assist clients with daily living activities, including personal care, meal preparation, and household management, as required.

2. Support Planning:
- Collaborate with clients and their families to develop and implement individualized support plans that reflect their goals, preferences, and needs.
- Regularly review and update support plans in collaboration with clients, ensuring they remain relevant and effective.

3. Community Engagement:
- Facilitate client access to community resources, services, and activities that promote social inclusion and personal development.
- Encourage and support clients in participating in recreational, educational, and vocational activities that align with their interests and capabilities.

4. Advocacy:
- Advocate for the rights and needs of individuals with disabilities, ensuring they have a voice in decision-making processes.
- Provide information and support to clients and families regarding available services and resources.

5. Collaboration:
- Work collaboratively with multidisciplinary teams, including healthcare professionals, therapists, and other service providers, to ensure holistic support for clients.
- Maintain open communication with families and caregivers to keep them informed of their loved ones' progress and any changes in support needs.

6. Documentation and Reporting:
- Maintain accurate and up-to-date records of client interactions, support provided, and progress made in line with agency policies and procedures.
- Prepare reports and documentation for funding bodies and regulatory compliance as required.

7. Professional Development:
- Participate in ongoing training and professional development opportunities to enhance skills and knowledge in the field of disability support.
- Stay informed about best practices, legislation, and policies relevant to disability services.

Requirements:

Certificate IV in Disability or a related qualification.

Proven experience working with individuals with disabilities in a community or support role (preferred).

Strong understanding of the National Disability Insurance Scheme (NDIS) and relevant legislation.

Excellent communication, interpersonal, and organizational skills.

Ability to work independently and collaboratively within a team.

A compassionate and respectful approach to supporting clients and their families.
