About the Role

Target 120 is an initiative that aims to work with at risk and recidivist young people and their families to reduce offending behaviour, thereby contributing to a long-term vision of creating safer and more connected communities.

The Family Mentor will work in partnership with the Program Leader and Family Support Worker. The Family Mentor will engage and support young people and their families in their home and/or community to explore and understand the young person’s aspirations to develop Individual Support Plans, assist inachieving individual goals and work towards achieving a supportive home life.

As a Family Mentor, your responsibilities include, but not limited to:

• Build and establish relationships with young people to develop individual goals to be included in Individual Support Plans.

• Provide strong outreach and case management support for young people to promote practical daily routine and stability, develop skills, and link to other services.

• Provide access to diversional and recreational activities for young people, including cultural and activities on country.

• Work collaboratively and effectively with Target 120 team and key community stakeholders to support families.

About You

The successful candidate will be highly organised and have strong communication skills together with the following:

• Relevant qualifications and/or experience working with the target group.

• A knowledge and sensitivity to the barriers that may prevent families and young people at risk from achieving their full potential.

• The capacity to manage a case load and implement individually tailored plans to address barriers.

• Sound organisational skills, including the ability to plan, prioritise and meet deadlines.

• The ability to work with minimal supervision.

• Intermediate/proficient level of IT and Administrative skills.

• Ability to communicate effectively, verbally and written.

• Effective interpersonal and relationship building skills.

• Act with integrity in all organisational activities and maintain confidentiality.

The successful candidate will need to hold a current WA driver's licence, Working with Children Check, and satisfactory National Police Clearance obtained within the last six months.

As part of our commitment to being a Child Safe organisation, successful candidates are required to provide their Immunisation History Statement. While immunisations are highly encouraged, they are not mandatory. This information will help us assess and implement appropriate safety measures for everyone.
