Location: Sonder Edinburgh North, Port Adelaide and other offsite locations as required

About the Role:
Join our multidisciplinary mental health team as a Clinical Care Planner, supporting individuals with complex mental health needs. In this role, you’ll coordinate care by working closely with patients, GPs, and other health professionals to develop tailored care plans. You'll play a key role in improving treatment outcomes through a stepped-care approach and providing professional consultation and liaison across services.
Experience Requirements:
- Demonstrated experience working within a fast-paced environment.
- A minimum 2 years supervised experience in the area of mental health.
- A minimum 2 years relevant professional development and or supervision.
- Experience in delivery cognitive behaviour therapy and other evidence based interventions.
Qualification and Other Requirements:
- Qualified in the following disciplines and membership to associated professional body; E.g.: Psychology, Social Work, Occupational Therapy, Mental Health Nursing or Health care professional
- Working towards or current and full registration with the appropriate registration authority
- Drivers Licence
- Current or willing to obtain a Working with Children's Check (WWCC)
- Current or willing to obtain National Police Clearance
