We are seeking a community-focused, enthusiastic, and highly motivated individual to join our team as a Home Support Coordinator.

In this rewarding role, you will be responsible for the development and coordination of our Social Options and Home Assist Programs, tailored to meet the needs of clients under the Commonwealth Home Support Program (CHSP).

Key Responsibilities:

Coordinate and deliver community care services in line with CHSP guidelines.

as part of the team, contribute to the development of Council’s CHSP Program, to meet the needs and enhance the wellbeing of clients

Ensure contracted outputs are met

Build strong relationships with clients, carers, service providers, and stakeholders.

About You:

To be successful in this role, you will bring:

Demonstrated knowledge of the issues and needs of the aged, people with disabilities, and their carers.

Experience in coordinating community care programs and managing government-funded services.

Excellent communication and organisational skills.

A proactive, compassionate, and community-minded approach.

This is a part-time contract position working Monday, Wednesday & Thursdays to 30 June 2027, with potential for extension subject to continued Commonwealth funding.
