About the Role

We are seeking a dedicated NDIS Disability Case Manager to join our Balcatta-based team. In this role, you will support NDIS participants living in Supported Independent Living (SIL) homes or in the community. You will oversee the coordination and delivery of services, supervise support staff, and ensure that quality, safety, and compliance standards are consistently met.
This is a full-time, Monday to Friday role (9:00am–5:00pm), with some flexibility required based on participant and service needs.
Key Responsibilities
Manage the onboarding process for new clients, including documentation and compliance.
Create and maintain Service Agreements in line with participants’ NDIS plans.
Maintain accurate and up-to-date client profiles in the CRM and MyPlace portal (for NDIA-managed clients).
Coordinate with support coordinators, allied health professionals, families, and internal teams to ensure consistent and safe service delivery.
Conduct regular home visits to monitor service delivery, site safety, incident management, and inventory checks.
Support financial performance of SIL sites by monitoring and managing expenditure within budget.
Supervise and support a small team of support workers, ensuring staff are trained, compliant, and responsive to client needs.
Oversee medication management, behaviour support plans, and incident reporting processes in line with the NDIS Quality and Safeguards Commission standards.
Prepare reports for NDIS plan reviews and contribute to ongoing service improvement.
Handle complaints and escalate issues appropriately to ensure participant satisfaction and service quality.
Ensure compliance with all NDIS and organisational policies, procedures, and documentation standards.
Eligibility Criteria
Certificate III in Disability, Individual Support, or a relevant qualification (desirable).
Experience supporting vulnerable individuals and helping them set and achieve personal goals.
Understanding of the NDIS and the NDIS Quality & Safeguarding Framework (or a willingness to develop).
Demonstrated ability to supervise daily service operations and ensure compliance.
Excellent verbal and written communication skills.
Strong problem-solving and conflict-resolution abilities.
Compassionate, participant-focused approach with a calm and professional demeanour under pressure.
