POSITION DESCRIPTION

Position Service Coordinator

Purpose

To collaborate with individuals and their families to identify their support needs and aspirations and implement supports that will meet the needs of the person and address their NDIS goals.

Coordinate service provision, recruit and provide guidance to Support Workers to meet the identified needs.

Award SCHADS Level SACS Level 5 Reports to General Manager Supervises Support Workers

General Responsibilities / Duties of All Staff

To ensure quality service, all FlexAbility staff will:

Present a positive image of people with a disability and the service to the public.

Foster a culture of service, learning, cooperation, compassion, and commitment to the values underpinning the mission, objectives, and service model of FlexAbility.

Develop and maintain effective, respectful working relationships with individuals and families supported by FlexAbility, other staff members and external stakeholders.

Maintain confidentiality of individuals and their families, other staff members and the organisation.

Prioritise their workload to ensure that the needs of the people served are met and take responsibility for continuous improvement and quality in their own work.

Work within the risk management framework while ensuring that individuals have choice and control and are afforded dignity of risk.

Comply with all workplace health and safety requirements.

Work in accordance with the mission, objectives, code of conduct, policies, procedures, and employee handbook of FlexAbility, as well as all relevant government legislation and standards.

Core Responsibilities of a Service Coordinator

Liaise with individuals and their families to identify needs and issues regarding service provision, develop and oversee the implementation of Support Plans.

Identify, analyse and mitigate risk while ensuring that individuals have choice and control and are afforded dignity.

Plan and carry out recruitment of Support Workers and develop rosters to ensure continuity of service as agreed with individuals and their families.

Provide modelling, mentoring, leadership and management to Support Workers to facilitate their success in their roles.

Maintain current knowledge of human rights and decision-making frameworks and the NDIS Quality & Safeguards Commission Standards, rules and requirements and ensure that support to individuals is provided in line with these.

Primary Duties of a Service Coordinator

Develop Support Plans and other necessary documentation based on discussion with the individual and their family to provide direct support staff with direction.

Coordinate day-to-day support provision including responding to requests or concerns from individuals and families and informing direct support staff of the day-to-day requirements of their position.

Make staff allocation decisions, ensure that the Individual Master Roster and Staff Master Roster spreadsheets are up to date and inform the Administration Assistant – Scheduling about any updates.

Liaise with the Administration Assistant - Scheduling to manage and maintain the active rosters.

Provide back-up to the Administration Assistant – Scheduling role as required including backfill of Support Workers as needed.

Maintain knowledge of, and abide by, award and industrial relations requirements when rostering or supervising staff.

Recruit Support Workers to ensure adequate staffing levels are maintained to provide continuity of care.

Mentor and supervise Support Workers to ensure they are performing their duties in line with FlexAbility policies and procedures and relevant legislation.

Identify and organise training required for Support Workers that is specific to individuals, liaising with the Administration Officer as needed.

Investigate, manage and follow up incidents, including uses of restrictive practices.

Develop and maintain records in line with FlexAbility procedures and NDIS requirements including Support Plans, operational notes, data collection, incident investigations, restrictive practice reports, and Service Provision Reports for Support Coordinators.

Other duties

Liaise with the Finance Officer regarding employee expenses when pays are being processed.

Liaise with and maintain working relationships with other service providers accessed by individuals.

Liaise with the General Manager, Plan Managers and Support Coordinators regarding individuals NDIS plan budgets and expenditure if consent has been given on the Consent Form.

Provide emergency cover for support shifts with individuals as needed.

Be responsible for the on-call phone as required.

Assist with the review and development service delivery procedures.

Other activities necessary to fulfil the responsibilities of the role.

Other duties as delegated by the General Manager.

Experience, Qualifications and Licences

Experience working in a leadership role in community services.

Associate Diploma in community support or other relevant tertiary qualification and/or equivalent experience.

Experience supervising staff.

High level of Computer literacy in Word, Excel, email and internet, ability to learn to use Microsoft Teams and customised databases.

Current Open C class Queensland Driver's licence.

Positive Notice Blue Card for Child Related Employment (apply on appointment).

NDIS Worker Screening approval (apply on appointment).

Documents to demonstrate the legal ability to work in Australia.

Selection Criteria

Demonstrated commitment to people who live with a disability, implementing human rights and person-centred approaches to service and the capacity to work developmentally with people and their families to achieve positive outcomes.

Demonstrated leadership skills including ability to exercise initiative, train and mentor staff, facilitate effective teamwork and evaluate performance of supervised staff.

Excellent interpersonal and verbal communication skills including abilities in negotiation, networking, advocacy, and conflict resolution.

Demonstrated ability to review data, analyse situations, plan, and prioritise to efficiently manage your workload and meet deadlines.

Demonstrated ability to identify issues, problem solve and manage resources.

Knowledge and understanding of continuous quality improvement, the NDIS Practice Standards and relevant legislation (including those relating to disability services, anti-discrimination, privacy, industrial relations, workplace health and safety) and how each applies to provision of service.

Advanced written communication and IT skills including the ability to write procedures.

Demonstrated work ethics of reliability, commitment, flexibility, diligence, and respect for colleagues and management.
