Rehability Australia is seeking an experienced Social Worker to join our Disability & Case Management Team, providing specialist disability and rehabilitation services to individuals living with Acquired Disabilities - Brain Injury (ABI) and Spinal Cord Injury (SCI).

About the Role

As a Social Worker in our Toowoomba team, you will:

Deliver specialist social work and case management rehabilitation services for clients with ABI and SCI.

Conduct psychosocial assessments and develop tailored support and rehabilitation plans.

Liaise with medical professionals, allied health providers, support coordinators, and community services to deliver high-quality outcomes.

Advocate strongly for clients to access appropriate funding, services, and resources across schemes including NDIS, NIISQ, icare, TAC, DVA, and other insurers.

Utilise Flowlogic CRM to manage client information and track service delivery effectively.

Build and maintain local disability sector connections to improve service accessibility for clients.

What We’re Looking For

Bachelor’s or Master’s degree in Social Work

Active membership with the Australian Association of Social Workers (AASW).

Minimum 2–5 years’ direct experience working with people who have sustained Acquired Brain Injury or Spinal Cord Injury.

Proven skills in social work, case management and rehabilitation planning.

Strong knowledge of Toowoomba’s disability services sector and established professional networks.

Ability to work independently while collaborating effectively within a multidisciplinary team.

Experience using Flowlogic CRM software.

A valid driver’s licence and reliable vehicle.
