About the role
Temporary full time opportunity to lead the RBWH Pharmacy Patient Flow Team, play a fundamental role in integrating Collaborative Pharmacist Medication Prescribing (CPMP) into clinical practice and to facilitate timely medicines management on discharge. This is the first in Queensland dedicated team for CPMP and requires a forward thinking leader to facilitate the evaluation and implementation of this service. The role provides clinical pharmacy expertise to the multidisciplinary team, ensures the delivery of a safe and effective Patient Flow service and contributes to the training and development of the pharmacy team.
This role works an 8-day fortnight across a 7 day roster, including extended hours.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Coordinate the delivery of the pharmacy Patient Flow Service including the provision of CPMP for high-risk admissions and coordination of Discharge medication management to optimise patient flow through RBWH. Support the evaluation of CPMP to demonstrate the clinical and economic benefits
- Undertake high level Pharmacy duties with infrequent need for direct clinical practice supervision including working outside of normal business hours and at weekends.
- Participate in collaborative pharmacist medication prescribing and advanced level decision making.
- Provide sound, high-level advice and assistance to clinical staff for evidence-based evaluation of new medicine therapies, and to professional and operational supervisors regarding service delivery.
- Work cooperatively and effectively with multidisciplinary teams within and external to pharmacy, and with patients/carers, and actively participate and provide high level advice within this multidisciplinary environment.
