About the Role
We're expanding and looking for an experienced, passionate NDIS Support Coordinator or Psychosocial Recovery Coach to join our growing team. In this hybrid role, you'll work closely with participants to implement and navigate their NDIS plans, build sustainable support networks, and achieve meaningful outcomes.
You'll be the go-to connection between participants, providers, and broader community services.
Key Responsibilities
Develop and review individualised support plans in collaboration with clients and their families.
Coordinate a diverse range of supports, including allied health, housing, employment, and community engagement.
Support participants to understand and effectively use their NDIS funding.
Liaise with service providers, NDIA representatives, and community stakeholders.
Monitor progress toward goals, and prepare reports to the NDIA (e.g., plan implementation and outcome reports).
Advocate for participant needs to ensure the best possible outcomes.
Maintain accurate and timely documentation in line with NDIS quality standards.
What We’re Looking For
20+ hours per week (can fit around school drop-off and pick-up)
Minimum 2 years’ experience in NDIS Support Coordination, Mental Health, Disability or a related role.
Tertiary qualification in community services, mental health, disability, or equivalent (Cert IV considered with experience).
Comfortable using computers and tech.
Strong understanding of the NDIS framework, pricing arrangements, and person-centred practice.
Excellent interpersonal, organisational, and advocacy skills.
Capacity to manage multiple caseloads with autonomy.
Proficiency with client management systems and basic tech tools.
Current Driver’s Licence and access to a reliable, insured vehicle.
Working with Children and National Police Checks, plus full Australian working rights required.
