About the Program/Team

The Youth Initiative (YI) is a growing service that provides support young people leaving statutory out-of-home care, aged between 16 and 21 years of age. The YI team work with young people to build their long-term capacity and resilience in order to permanently divert them from the homelessness service system.

Personal Advisors work with young people transitioning out of Permanency Support (Out of Home Care) who do not have established networks, for up to 3 years. The role includes working with young people to implement their leaving care plan, establish the life-long habit of growing and maintaining their support networks, navigate mainstream and specialist services, and manage crisis and change effectively.

Key Responsibilities

Build strong, trusting relationships with young people aged 18–25 as they transition from Out of Home Care into adulthood.

Deliver trauma-informed, strengths-based support that helps young people identify their goals and build independence.

Act as a consistent, reliable Personal Advisor—supporting young people for up to 3 years across key areas like housing, education, employment, and wellbeing.

Help young people navigate services such as mental health, AOD, and community programs—while empowering them to advocate for themselves.

Support young people into safe, stable accommodation, including developing the skills they need for independent or shared living.

Lead regular outreach and review meetings with clients and service partners to ensure supports remain relevant and effective.

Collaborate with housing providers, community services, and local networks to create meaningful opportunities for young people.

Keep accurate case notes and contribute to service improvement through data collection and reflection.

Selection Criteria

The ideal candidate will bring:

A Cert IV or higher in Community Services, Youth Work or related field (or working towards a Diploma/Degree).

Experience supporting young people from Out of Home Care using trauma-informed, strengths-based practice.

Skilled in mentoring, goal setting, and care planning with young people who have complex needs.

Strong knowledge of youth services, care-leaver rights, and after-care supports.

Confident in building partnerships with a wide range of service providers and stakeholders.

Excellent organisation and communication skills, with the ability to travel regularly across Western NSW.
