About the role

AB Nurses Support Services is seeking a passionate and dedicated Support Coordinator to join our team in Pakenham, VIC. This is a casual role that will play a vital part in providing holistic support to older adults and individuals living with disabilities within our local community.

What you'll be doing

• Coordinate and implement individualised support plans to meet the unique needs of each client
• Facilitate access to a wide range of community services and resources to enhance the independence and wellbeing of clients
• Collaborate with clients, their families, and other healthcare professionals to ensure seamless service delivery
• Monitor client progress and adjust support plans as required
• Maintain detailed records and report on client outcomes
• Identify opportunities to improve service quality and efficiency

What we're looking for

• Relevant tertiary qualification in a community services or allied health discipline
• Minimum 2 years' experience in a similar support coordination or case management role
• Excellent communication and interpersonal skills with the ability to build rapport with diverse clients
• Strong problem-solving and critical thinking skills to navigate complex situations
• Demonstrated understanding of the aged care and disability support sectors
• Proficiency in developing and implementing individualised support plans
• Commitment to upholding the rights, dignity and independence of clients
• Flexible and adaptable approach to work
