About the role

We are seeking a resilient and empathetic Community Mental Health Practitioner for a fixed-term, part-time role (average 25 - 30 hours per week) at our short-stay Step-Up Step-Down (Kwelena SUSD) residential service in Rockingham. This recovery-focused mental health service, delivered in partnership with Southern Metropolitan Health Service and Housing Choices, supports adults aged 18 to 64. Kwelena SUSD provides a safe, therapeutic environment that offers a ‘step-up’ alternative to hospital admission or a ‘step-down’ pathway from hospital back into the community.

In this role, you will deliver intensive person-centred, recovery-oriented support to help people work towards their wellbeing goals, develop independent living skills and daily routines, build confidence to live safely in the community, and strengthen social and community connections. This role requires flexibility to work a 24/7 rotating roster, including night shifts, weekdays, weekends, and sleepovers.

Key responsibilities

  • Support people to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
  • Purposefully engage with people using techniques including: Brief intervention, Motivational interviewing and coaching, Family inclusive practice, Trauma informed practice, Conflict resolution.
  • Plan and deliver group programs that aid people to build their skills and focus on recovery.
  • Support family and carer roles through understanding their concerns and the provision of information, education and referrals.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to people’s recovery goals.
  • Contribute to the day to day operations of the residential service through undertaking a range of housekeeping duties.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Minimum 12 months’ experience working in a similar mental health role.
  • Hold a current First Aid and CPR certificate and perform these duties as required as part of the role.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
