Nurse Next Door Sunshine Coast is looking for an experienced RN to join our growing home care team in a Care Designer Role.

You will be responsible for client oversight and team management in your local area on the Sunshine Coast which is part of a much larger territory covering the Sunshine Coast, Noosa, Gympie and Tin Can Bay overall.

The Care Designer is a senior RN and is integral to the Nurse Next Door team.
They are responsible for the coordination and management of community client care to ensure the best possible client outcomes. The primary objective of a Care Designer is to ensure that clients receive care of the highest quality, accompanied by WOW customer experiences. Working in collaboration with the Franchise Partner or Manager, Care Designers provide support in overseeing and coordinating the complete caregiving team, which includes Care Ambassadors, nurses, and caregivers.

The successful candidate will be passionate about making a difference, a fast learner, detail oriented, self driven, positive and adaptable. A high level of organisation and follow through is essential. Competence in various administrative tasks is essential -spreadsheets, email, scheduling, client & team communications etc.

Key Responsibilities of the Care Designer include (but not limited to):

Attend and complete Caring Consults

Follow up on local intake enquiries and those directly from HeartQuarters

Supervisory visits, practical assessments and care plan reviews every 60 days

Attend nursing visits and assessments, report writing

Write Client Care Plans

Liaise with external client care teams including support coordinators, medical & allied health

Conduct First visits and practical team assessments

Schedule ongoing client visits for your area and urgent vacant visits

Monitoring various team chat groups

Maintaining documentation of client and team member files

Manage and nurture client relationships to build strong connections

Manage and nurture care team relationships to build strong connections

Conduct nursing and caregiving team training

Daily visit approval ensuring accuracy for payroll

Manage and nurture support coordinator/case manager relationships to build strong connections

Daily review of client progress notes & dashboard

Complete form approvals including but not limited to: Wound Care Reports, Catheter Reports - ensuring all reports are completed correctly and that all processes are correctly documented and signed off.

Incident Reports - ensuring all incidents are reported correctly, investigated promptly and risk assessments are completed

Keep up to date with information released from HeartQuarters

Provide direct and indirect support to our team of nurses and caregivers including delegating administrative tasks where required

Review and sign off on all delegated tasks including care plan writing and clinical assessments

Attend Discovery Day -meeting new caregivers and nurses and guiding through various components of new team onboarding

Assuming the responsibilities of other Care Designers and Care Ambassadors taking leave
