Description: Join our team as a Cancer Support Worker and provide practical day-to-day support to community members navigating the health and community support systems involved in the cancer journey.
Key Responsibilities:
Service Delivery:
Increase community awareness by working with the local health promotion team.
Develop auditing and reporting tools to identify cancer cases for follow-up.
Establish relationships with mainstream health services to ensure culturally appropriate services.
Accompany community members to cancer screening and diagnostic testing appointments.
Conduct new staff orientation and internal training on cancer signs, symptoms, treatment options, and post-surgery care.
Develop care plans/journals for community members with cancer.
Provide ongoing supportive care, ensuring access to medicines, food, and other necessities.
Deliver peer support networks in collaboration with the Health Promotion Officer.
Assist patients in navigating the healthcare system, including scheduling appointments and understanding treatment plans.
Offer emotional support and counselling to patients and their families.
Advocate for patients' needs and rights within the healthcare system.
Collaborate with healthcare providers to ensure comprehensive and coordinated care.
Develop and maintain relationships with community organizations and support groups.
Monitor patient progress and adjust care plans as needed.
Provide regular reports on patient outcomes and program effectiveness to the management team.
