ABOUT THE ROLE

The Assertive outreach program is a dynamic way of organising and delivering highly coordinated, flexible case management support and healthcare to people in the community who are in-between Housing and predominantly rough sleeping - It requires working with people respectfully in their environment, meeting people where they are at, and working towards intergrading people back into community and towards rapid Housing pathways while providing advocacy and referral to a wide range of multidisciplinary supports.

This is a fixed term full time role to June 2026.

Criteria:

• Qualification in Community Services or other related field at Cert IV minimum.
• Current unencumbered Driver’s Licence
• Significant experience in an associated field
• Understanding and experience of working with Aboriginal people and their communities.
• Demonstrated experience in case planning and coordination of people with complex support needs and within marginalised communities.
• Understanding of, and demonstrated ability to work with Human Service Agencies.
• Demonstrated experience and or understanding of Trauma Informed Care.
