We are looking for experienced support workers (Lifestyle Facilitators) to provide in home support to a young mum and her daughter in their family home.

As a Lifestyle Facilitator, no day is the same. You will be assisting this lady to be the best mum to her daughter as independently as possible, You will helping her to achieve her personal goals this will mean providing support in areas of daily living, social support, and personal care.

The successful applicant will:

Possess a genuine passion for working with people

Be reliable, flexible, and committed to assisting people to achieve their goals and live their best life.

Have great availability and be able to work a set roster with capacity to assist in covering annual and sick leave.

Ability to work with children as you will be working with a child who is under 5 years.

Ability to work in a fast paced environment.

Maintain exceptional professional standards of care and support

Have own transport to get to and from your shift.

Competent in driving a wheelchair accessible vehicle.

Available shifts 7 days per week are as follows

6am-8am

6am-11am

6-8pm

10pm-6am- Active night

Key Selection Criteria

Certificate III or IV in Disability, Individual Support or nursing experience is highly desirable but not essential. Personal experience will also be considered.

Demonstrated exceptional communication skills and ability to be able to complete records and documentation accurately.

Be able to follow instructions and follow policies and procedures effectively.

Willingness to work as part of a team and at times, independently.

Sound Knowledge of the NDIS practice standards.

Current First Aid and CPR certificates

Current NDIS Worker Screening (yellow card) and Working with children Check (blue card) - or willingness to obtain.
