About the Role:

We’re currently expanding our multidisciplinary team and seeking an Accredited Practicing Dietitian to deliver person-directed dietetic support. Your primary responsibility will be conducting telehealth consultations, supporting clients through education toward self-management, and empowering them to make informed decisions about their health. This position offers a unique opportunity to make a meaningful impact on the lives of veterans and their families while working remotely.

Key Responsibilities:

Dietetic consultations: Provide person-directed dietetic education and support across a range of areas related to veteran’s chronic health conditions including diet, nutrition, medication and lifestyle.

Person-directed care: Ensure that dietetic plans are person-centred, taking into account each individual’s unique goals, preferences and literacy.

Telehealth Consultations: Conduct individualised telehealth (phone or video) consultations with DVA recipients for assessment, completion of a patient care plan and implementation dietetic support.

Multidisciplinary Co-ordination: Refer clients to our extensive network of GPs, Psychologists, Psychiatrists, Diabetes Educators, Exercise Physiologists, Podiatrists and to Case Conferencing (with GP and multidisciplinary team), as needed.

Empowerment: Practise evidence-based health coaching techniques (E.g. Motivational Interviewing) to empower clients to take control of their health by delivering content, providing guidance on self-management and decision-making to achieve positive health outcomes.

Documentation: Maintain accurate and detailed patient records contemporaneously for clients’ progress notes, patient care plan, end of cycle report, and DVA claim item numbers for every consultation, every time.

Stay Informed: Stay up-to-date with the latest advancements in condition management, treatment options and educational resources to continue to contribute to the development of our dietetics service.

Compliance: Ensure dietetic services provided align with DVA guidelines, and relevant professional standards and codes of conduct.

Requirements:

Current Dietitians Australia registration as an Accredited Practicing Dietitian.

Excellent communication skills and ability to establish and maintain therapeutic client relationships

Empathy and a person-centred approach to care

Excellent documentation and record-keeping abilities

Ability to deliver dietetic support via telehealth

Ability to work independently and remotely as part of a national network of multidisciplinary services
