About us
Hi there! We're Five Good Friends, and we're an approved aged care and registered NDIS service provider. We help our clients (we call them Members) live vibrantly in their own homes by wrapping a care team around them and their families. Our software coordinates in the background so our teams can do their best work. Our carers (we call the Helpers) play a key role in creating the magic for our members. Five Good Friends strives to form high-quality, long-lasting connections between Members and their Helpers.
About the job
As a Community Registered Nurse you will visit and support people and their loves ones who are looking for the right provider of their care and support. You’ll help them understand how they can use their funding with Five Good Friends, and how we can work together to support them to live the life they choose. Travel is a large part of this role, and we do offer reimbursement for KM's travelled on the job.
You will work together to understand what is important to our members, what they need and want to stay living at home, and begin to plan how we can support this, collaborating with the team of Coordinators, and the carers (Helpers).
The day-to-day ranges from:
Provide clinical oversight and support for Members with complex care needs, including home visits and risk assessments.
Collaborate with Care Teams to ensure high-quality, person-centred support across our communities.
Referrals to allied health professionals and external providers to coordinate care and services.
Contribute to the review of Help Plans, ensuring Helpers well-informed.
Support complaint and incident management processes in line with quality and safety standards.
Assist in maintaining compliance with relevant legislation and contribute to successful industry accreditation.
Work with the Manager - Complex and Clinical Care on the development and implementation of clinical service improvements.
Provide timely, practical clinical advice and support across the business to enhance care outcomes.
Be curious and look for new ways to improve process, grow our brand locally and deliver value back to our partners.

Experience / Qualifications
Formal qualifications in nursing required (AHPRA Reg)
Current or recent community, aged or primary care nursing would be highly regarded, however, we know how broad nursing is and how great many different people can be
Strong and genuine interpersonal, oral and written communication and negotiation skills are important - you need to be able to talk with and support many different people, and handover top-quality information to your colleagues
Understanding and experience in providing excellent customer service is valuable to us
Experience in Wound and Catheter management is advantageous.
Passion for supporting older Australians to live meaningful and independent lives.
Ability to develop strong and trusting relationships with a wide range of stakeholders.
Excellent communication and interpersonal skills.
High level of integrity, confidentiality, and professionalism in all interactions.
Current driver’s license and National Police Check.
You must have the right to work in Australia.
