About Us?
The Institute for Urban Indigenous Health (IUIH) leads the development, planning and deliver of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.
The Integrated Team Care (ITC) program aim is to improve access to the most appropriate health services for Aboriginal and Torres Strait Islander people at the right time and in the right place with a view to reducing avoidable acute hospital admissions or readmissions and emergency department presentations; improved use of the healthcare system and to support people to take part in a healthy lifestyle.
About the Role?
As an Indigenous Outreach Worker, you will be required to support effective improved access for Aboriginal and Torres Strait Islander People to available health care services in the regions. This will include liaising with local Indigenous Communities/Groups, identification of barriers to access and provide practical assistance to identified Aboriginal and Torres Strait Islander peoples to manage primary health care related needs.
Day to day duties will involve:
- Transport and assist clients with attending GP consultations, care coordination appointments, including first and follow up appointments, and non-health-specific needs (such as Centrelink, employment, social health needs)
- Assist with deliveries of essential items organised through Mob Link
- Assist with deliveries of equipment and supplies organised through Mob Link
- Establish and maintain effective and continuous cooperative working relationships with Indigenous community organisations
- Initiate and document own community events and activities, as well as participate in community events and activities organised by others
- Establish strong cultural ties and links with local Aboriginal and Torres Strait Islander communities to encourage and support the increased use of health services
- Collate and maintain a library of relevant service provider related materials including service registration processes and forms, and information about partner GP Practices, other mainstream service providers, IUIH programs and clinics and relevant non health social service providers and services
- Contribute to improving processes/systems of partner GP practices
- Keep accurate, current case notes, statistics and other client records and files, as required
About You?

Essentials:
- Current QLD Drivers Licence
- Valid QLD Blue Card (Working with Children) Must have prior to commencement
- Valid Disability Worker Screening Check (Yellow Card) Must have prior to commencement
- Current CPR and First Aid Certificate
- Criminal History Check
Desirable, but not mandatory:
- Previous experience working with Aboriginal and Torres Strait Islander people and /or communities
