About the role

Freesia Care is seeking an experienced and dedicated Community Nurse to join our team in Tweed Heads and surrounding areas, NSW, with occasional coverage of the Gold Coast region, QLD. This is a casual role, providing you the flexibility to balance your work and personal commitments. As a Community Nurse, you will play a vital part in delivering high-quality, compassionate healthcare to the local community.

What you'll be doing

Providing comprehensive nursing care to patients in their homes and within the community

Assisting with the management of chronic conditions and supporting patients with their daily living activities

Collaborating with multidisciplinary teams to develop and implement individualized care plans

Advocating for the rights and needs of patients and their families

Maintaining accurate and up-to-date patient records

Participating in continuous professional development to enhance your nursing skills and knowledge

What we're looking for

Registered Nurse with a current Australian nursing registration

Minimum 1 year's experience in a community nursing or similar role

Strong communication and interpersonal skills to effectively engage with patients and their families

Excellent organisational and time management skills to manage a diverse caseload

Commitment to providing high-quality, patient-centred care

Ability to work autonomously and as part of a multidisciplinary team

Proficiency in using relevant nursing documentation and information systems
