WHO ARE WE?
Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
ABOUT THE ROLE
The Population Health Team Leader will provide leadership and support to service wide public health programs including Capacity Building Portfolios. They will liaise with senior and clinic managers across DDHS Integrated Clinical Services and Programs to support the delivery of community education and promotional events across Danila Dilba Health Service.
WHAT YOU WILL DO
- Develop the DDHS Population Health Strategy including quality improvement within the clinics and community education
- Lead the design of community and staff consultations with the support of the Aboriginal Cultural Lead and Health Education and Engagement Lead
- Support and provide leadership for service wide clinical education training across DDHS
- Support the Population Health team with the following quality improvement activities for each portfolio within the Danila Dilba Clinics:
- Communicare
- Data Quality and Reports
- Clinic Flow
- Clinician Education
- Client Journey and Engagement
- Ensure a Continuous Quality Improvement approach is taken within identified Population Health priorities
- Support service wide health promotion events, alongside communications team and Health education and engagement team.
- Support the Health Education and Engagement lead in the coordination, monitoring, implementation and evaluation of the Tackling Indigenous Smoking program
- Provide expert advice, project oversight and support for service wide public health portfolios such as the Rheumatic Heart Disease and STI/BBV projects.
- Contribute to effective clinical governance arrangements that ensure high levels of quality and safety.
- This role will be required to work outside of standard business hours to support community events.
SELECTION CRITERIA
Essential
- Tertiary qualification related to Public Health or Health Professional Qualification (including Aboriginal Health Practitioner)
- Minimum 2 years' experience Public Health/Population Health Coordination
- Minimum 2 years' leadership experience including people and contract management.
- Experience in consulting with Aboriginal and Torres Strait Islander Community and peers
- Demonstrated ability in use of program logic, planning, managing, implementing, and evaluating projects.
- Well-developed interpersonal, facilitation and negotiation skills
- Demonstrated skills in supervising and supporting staff working in Health Promotion and collaborating with internal and external stakeholders.
- Experience in project management
- Well-developed formal written and verbal communication skills along with an ability to utilise appropriate data bases and other computer programs.
- Demonstrated knowledge of the Aboriginal Community Controlled primary health care sector and the role of quality in primary health.
Desirable
- Experience with communicare and/or working experience in Aboriginal Community Controlled Health Organisation
- Program evaluation and data collection experience.

EMPLOYMENT IS SUBJECT TO
- Willing to undergo a Police Check.
- Ability and willingness to obtain a Working with Children Clearance (Ochre Card).
- Current driver's licence.
- Current CPR certificate or the preparedness to gain one within 6 months of commencement.
- Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR) be submitted.
