About the role:

The MyStep to Mental Wellbeing Team Care Clinician provides evidence based psychological intervention and care coordination for consumers with severe and persistent mental illness, who are not actively supported by a Community Mental Health Team.

This may include working with consumers with varied needs including mental illness, disorder or dysfunction, consumers with emotional difficulties and experiencing life transition, distress and/or crisis.

The Team Care Clinician will provide consumer focused, trauma-informed, goal oriented clinical care and psychological interventions across the MyStep to Mental Wellbeing Program.

Given the complex needs of this consumer group, the clinician will work collaboratively with carers, families and relevant agencies including; GPs, Psychiatrists, Allied Health providers, non-Government organisations and community organisations to deliver effective and integrated care to meet the goals and needs of the consumer.

This role may involve some outreach to alternative sites within the district and will involve the delivery of flexible therapy a range of methods including face-to-face and telehealth.
