Position Summary
As our NDIS support Facilitator , you’ll manage the coordination of disability support services initiatives across Townsville and surrounding regions. You will ensure quality and compliance in service delivery, build strong community and clinical partnerships, and support the expansion of Orange Health Care's presence in Northern Queensland.
This is a dynamic leadership role requiring a balance of care coordination, compliance knowledge, stakeholder engagement, and rostering efficiency.
Key Responsibilities - NDIS support Facilitator
· Comply with the NDIS Code of Conduct to support participants in ways that are respectful and inclusive and are consistent with person-centred practices.
· Manage participant onboarding, intake, and assessments across NDIS.
· Develop and implement individualized support and care plans in alignment with funding requirements and personal goals.
· Support participants to build the confidence and skills to develop social networks and connections.
· Coach and build confidence in participants to develop the skills they need, such as activities of daily living, social etc.
· Coordinate clinical, personal, and domestic services in collaboration with GPs, families, allied health, and internal teams
· Ensure documentation and service delivery meet national standards and legislative compliance
· Apply principles of positive behavior support when supporting participants to implement their behavior support plan (as applicable).
· Provide debriefing, practice leadership, coaching and feedback on positive behavior support to team workers.
· Handle escalations, risk assessments, and critical incidents with sensitivity and professionalism
· Oversee care delivery for participants requiring high intensity support or complex care
Rostering & Service Delivery:
· Manage staff rostering to match worker capabilities with participant needs and preferences and program budgets (in consultation with Team Leader).
· Collaborating with the General Manager and HR on recruitment and induction of support staff.
· Provide ongoing mentoring and support to Team Leaders and frontline workers
Business Development & Community Engagement:
· Build and maintain strong professional networks with key stakeholders, government bodies, and referrers.
· Support local marketing initiatives and content development alongside the central team
· Track service metrics, monitor regional KPIs, and contribute to operational planning
· Contribute insights into service expansion opportunities across NDIS and other programs.
Qualifications & Experience
· Minimum 2 years of experience in community, disability (NDIS), or aged/veteran care coordination
· Knowledge of NDIS Quality & Safeguarding, and rostering systems
· Experience coordinating teams, managing care plans, and handling compliance/reporting
· Valid Yellow Card, Blue Card, First Aid & CPR, and current Driver’s License with insured vehicle
Desirable:
· Diploma or Bachelor’s in Disability, Nursing, Human Services, Social Work, Business, or related field
· Experience in business development or service expansion
· Understanding of Rostering processes and Community.
· In-depth understanding of statutory roles and disability-related legislation (NDIS).
· Exceptional written and verbal communication abilities.
· High level of resilience, empathy, and strong relationship-building skills.
· Experienced in conducting and representing in stakeholder meetings
