About you
Do you hold a current Registration with the Occupational Therapy Board of Australia; or Bachelor of Social Work and eligible for membership to the Australian Association of Social Workers; or Current Registration with the Psychology Board of Australia; or Current Registration with the Nurses and Midwives Board of Australia; or current registration with the Physiotherapist Board of Australia? Can you demonstrate an ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care?
What you'll be doing
The role will provide high level input, information, advice, referral and care planning for aged care patients, families, carers, acute and community services involved in the care of elderly people with age related health issues and complex needs. In addition, the role will be responsible for the provision of aged care assessments of patients in line with MNCLHD Aged Care Assessment Team guidelines.

Mid North Coast Local Health District (MNCLHD) covers an area of 11,335 square kilometres extending from Port Macquarie Hastings Local Government Area in the south to Coffs Harbour Local Government Area in the north.
