The Role
As our Activity Coordinator, you will play a pivotal role in organising and overseeing the Lifestyle teams in our four facilities across Victoria. You will lead these teams in enhancing the emotional, physical, and social wellbeing of our residents through tailored recreational and therapeutic programs. You will be responsible for designing, facilitating, and evaluating lifestyle activities that reflect the diverse interests, abilities, and cultural backgrounds of our residents.
Key Responsibilities:
Identify and apply key knowledge around our four diverse communities to tailor activity programs specific to these very distinct localities.
Lead Hope Aged Care’s four Lifestyle Coordinators and their teams to develop and maintain a monthly calendar of varied activities, events, and outings based on resident interests, abilities, and clinical needs.
Deliver person-centred programs that support cognitive stimulation, physical activity, reminiscence therapy, arts and crafts, music therapy, intergenerational projects, spiritual services, and community engagement.
Collaborate with nursing and care staff to integrate lifestyle activities into each resident’s individual care plan.
Encourage and support resident participation in all activities, including those with limited mobility, cognitive impairments, or complex care needs.
Coordinate and supervise volunteers and external entertainers or community groups.
Ensure each facility is in compliance with Aged Care Quality Standards in the planning and delivery of programs.
Track participation rates, collect resident feedback, and continuously evaluate program effectiveness.
Prepare reports and maintain accurate documentation, including resident notes, risk assessments, and incident reports.
Work closely with families to involve them in the lifestyle program and identify meaningful engagement opportunities.
Work closely with marketing departments to track and highlight a true reflection of the positive effects diversional therapy has within aged care communities
Required Qualifications & Experience:
Minimum qualification: Diploma in Community Services, Leisure and Health, Aged Care, or a related discipline (Essential)
Relevant experience in a similar role within residential aged care, community care, or disability services.
Strong understanding of the physical, cognitive, and emotional needs of older people, including those living with dementia.
Strong leadership skills with a high level of empathy, patience, and cultural sensitivity
Demonstrated ability to plan, facilitate, and evaluate recreational programs in line with evidence-based practice.
Excellent interpersonal, communication, and conflict resolution skills.
Proficiency in Microsoft Office and electronic documentation systems (e.g., LeeCare or similar)
Proven experience in coordinating or overseeing multiple sites or teams (desirable)
Strong knowledge of aged care standards, person-centred care, and therapeutic recreation.
Excellent leadership, organisational, and time-management skills.
High-level interpersonal and communication skills, with a focus on empathy and rapport-building.
Ability to work autonomously while maintaining collaboration with facility staff and management.
Competency in documentation, care planning, and digital record-keeping systems.
Current First Aid and CPR certificate
National Police Check and Working with Vulnerable People clearance (or willingness to obtain)
Willingness to travel between sites or comfortably work with sites remotely
Current First Aid and CPR Certificate
Valid National Police Check and NDIS Worker Screening Check (or willingness to obtain)
