The Role
A State Government Agency is seeking a Case Manager - Recovery & Support for an initial 12 month contract, based in Geelong. You'll manage a portfolio of injured individuals and will be on supporting recovery and return-to-life outcomes through active engagement with individuals, their networks, and service providers.
Responsibilities
- Coordinate recovery plans and manage a caseload of complex cases
- Provide empathetic support and proactive communication with injured individuals
- Engage with support networks, service providers, and internal stakeholders
- Identify services and programs that improve recovery and social outcomes
About You
- 2+ years' case management experience in personal injury, health, or disability
- Comfortable supporting sensitive and complex cases
- Understanding of recovery models, support services, and return-to-work strategies
- Tertiary qualifications in allied health, rehabilitation, or related fields are highly desirable
