This is a Recruitment Pool for Permanent Full Time, Permanent Part Time, Fixed Term Full Time, Fixed Term Part Time, with the possibility of extension(s) and/or permanency.
Appointments may be made from this pool until 30/11/2025.
About the OPPORTUNITY
In this role you would provide education and support to Chronic Kidney Disease (CKD) clients and their families, support the CKD client and their family to prevent progressions towards End Stage Kidney Disease (ESKD) and improve health outcomes through education and support in an outpatient setting.
To be successful in this role, you will have…
- Experience in Chronic Kidney Disease process management and education.
- Knowledge of primary, secondary and tertiary prevention as it applies to Chronic Kidney Disease management.
- Well developed interpersonal skills and ability to communicate across a diverse spectrum of the population.
- An ability to work within a multiâ€disciplinary team framework.
- An understanding and respect for the needs of Aboriginal culture and providing effective health service delivery to Aboriginal people.
- Eligible for / or in possession of a current C or C-A class drivers licence with the ability to travel within the region as required including overnight stays

The Country Nursing & Midwifery incentive program is available to new and existing nurses and midwives - across a broad range of patient-facing roles - on fixed term or permanent contracts at eligible locations. Eligible nurses and midwives can receive between $5,000 and $17,000 in support over 12 months.
