We offer a comprehensive suite of services, including in-home care, transport assistance, community participation, support coordination, cleaning and house or yard maintenance and respite care.

Key Responsibilities

Facilitate the end-to-end onboarding of new participants, ensuring all intake processes and documentation are completed accurately and promptly

Draft, update, and manage NDIS Service Agreements in line with participant plans

Input participant data, profiles, and funding streams into our system, maintaining data integrity

Provide administrative oversight, staff supervision, and coordination of direct support hours

Liaise with stakeholders including Support Coordinators, participants, families, GPs, pharmacies, OTs, physios, and other allied professionals to ensure continuity of care

Conduct home visits to monitor service delivery, participant wellbeing, site safety, safeguarding practices, and inventory management

Support financial performance by managing expenditure in line with approved budgets for participant support packages

Align participant supports with individual NDIS goals, including assistance with daily living skills and personal care

Lead a small rotating team of support workers through supervision, training, and issue resolution, ensuring consistent, quality service delivery

Ensure all staff are aware of and act on individual client care requirements

Promote accurate and timely documentation, including progress notes, case notes, and reportable records

Monitor medication administration practices and ensure compliance with policy

Ensure all client assessments are completed and updated regularly to remain aligned with the NDIS Practice Standards

Record, report, and escalate incidents and restrictive practices in accordance with internal procedures and Commission requirements

Draft reports for NDIS plan reviews, and contribute to planning and goal-tracking processes

Act as a key problem solver, ensuring consistent service delivery and addressing any service disruptions or challenges promptly

Maintain full compliance with NDIS Quality and Safeguards Commission standards and our internal policies

Participate in complaint resolution, aiming to deliver high participant satisfaction and ongoing quality improvements

Required Skills & Experience

Certificate III or above in Disability, Individual Support, or a relevant qualification

Sound knowledge of the NDIS Quality and Safeguards Framework and person-centred practice

Proven experience in service coordination, support delivery, support coordination and team supervision within a disability or human services setting

Strong written and verbal communication skills, with the ability to engage stakeholders effectively

High attention to detail and strong administrative skills, particularly in software and NDIS systems

Ability to remain calm under pressure, resolve conflict, and manage competing priorities

A compassionate and client-focused mindset with a commitment to upholding human rights
