About the role

As a Support Coordinator at Easy HealthCare Pty Ltd, you will play a vital role in coordinating and advocating for the provision of services to support people with disabilities or aged care needs. Working in a fulltime capacity from our Laverton North, VIC 3026 office, you will be responsible for ensuring clients receive the personalised care and assistance they require to lead independent and fulfilling lives.

What you'll be doing

Assessing client needs and developing tailored support plans

Coordinating the delivery of services from a range of providers to meet client goals

Advocating on behalf of clients to secure appropriate funding and resources

Regularly reviewing client progress and adjusting support plans as required

Building relationships with clients, their families, and service providers

Maintaining detailed client records and reporting on outcomes

Collaborating with the broader team to ensure a cohesive approach to client care

What we're looking for

Tertiary qualification in a relevant field such as social work, mental health or disability studies

Extensive experience in a similar support coordination or case management role

Excellent interpersonal and communication skills, with a client-focused approach

Strong problem-solving and organisational abilities to juggle multiple priorities

Solid understanding of the aged care and disability support sectors and funding mechanisms

Empathy, patience and a genuine passion for making a positive difference in people's lives
