About the Role

As a Level 3 Support Coordinator, you will:

Manage complex caseloads, delivering high-quality coordination for participants with intensive support needs.

Lead hospital discharge meetings, working alongside hospital staff, NDIA Hospital Liaison Officers (HLOs) and allied health professionals to ensure safe, well-planned transitions home.

Collaborate with hospitals, mental health services, housing providers and community agencies to create integrated, sustainable support plans.

Identify and mitigate risks, respond to crises and resolve barriers to care.

Build participants’ capacity for self-advocacy, decision-making and independent living.

Source, negotiate with and coordinate providers that align with each participant’s goals and funding plan.

Oversee plan implementation, monitor progress, and adjust supports as needs evolve.

Prepare detailed reports, discharge summaries and crisis logs in line with NDIS Quality and Safeguards Commission standards.

Act as the key liaison between participants, families, service providers and NDIA planners to ensure cohesive, effective supports.

About You

Minimum 2+ years’ experience in Level 3 (Specialist) Support Coordination, with a proven record in complex case management and hospital discharge planning.

Diploma in Community Services or above—or qualifications in Social Work, Allied Health, Nursing or a related discipline. Allied Health registration is highly regarded.

Deep understanding of the NDIS framework, legislation and Level 3 coordination standards.

Strong professional connections across health, mental health, housing and disability services in Melbourne.

Excellent communication, problem-solving, advocacy and risk management skills.

Valid NDIS Worker Screening Check, full Driver’s Licence, reliable vehicle and full working rights in Australia.
