Position Title: Community Worker

Job Location: Manunda, Cairns

Position Type: Full-Time ( 38 Hours per week)

Remuneration: $75,000- $80,000

About the Role:

We are currently looking for a compassionate and proactive Community Worker to become a vital part of our team. In this role, you will be responsible for evaluating client situations, designing and implementing tailored support initiatives, and helping individuals access the resources they need to thrive. You will also play an important part in community engagement, resource mapping, and inter-agency collaboration to address local needs and challenges.

Duties and Responsibilities:

Conduct thorough assessments to understand the unique needs of individuals within the community, and develop tailored care plans, educational resources, and support programs that foster self-reliance, dignity, and personal development.

Engage clients in empathetic, structured conversations to explore their social, emotional, and practical challenges, and collaboratively design care pathways and achievable goals for positive change.

Monitor each client’s journey by evaluating their progress against established care objectives, and maintain clear, confidential records and outcome reports for both internal team use and external reporting requirements.

Facilitate timely access to specialised services by connecting clients with external providers, health professionals, or welfare agencies that can offer targeted assistance beyond the organisation’s scope.

Conduct broader community assessments to determine gaps in essential services—such as housing, health care, employment, and training—and support the development of community care initiatives that address those systemic needs.

Build and maintain collaborative relationships with local service providers, community groups, government bodies, and private stakeholders to promote integrated care and improve accessibility to community-based services.

Provide compassionate support to families and vulnerable individuals including children, people with disabilities, and those facing social isolation by delivering care in home-like environments, shared accommodation, or residential care facilities, depending on their needs.

Supervise and support community members who are under legal supervision, such as probation or parole, ensuring they comply with conditions while helping them reintegrate through consistent guidance and access to care networks.

Offer practical and emotional support to young people experiencing hardship, including assistance with housing, relationships, and financial stress, while empowering them through mentoring, education, and referrals.

Assist in securing funding for community care projects by preparing well- researched grant applications, formal proposals, and progress reports to submit to government agencies, philanthropic organisations, and partner institutions.

What We’re Looking For:

Previous experience working in community engagement, case management, or social support roles.

Strong verbal and written communication skills, with the ability to engage sensitively with people from all backgrounds.

Demonstrated experience in supporting individuals from refugee backgrounds coping with migration-related trauma.

Exceptional time management and documentation skills.

A good understanding of the local service landscape, including referral networks and community programs.

Essential Requirements:

NDIS Worker Check

A current Australian Driver’s License

National Police Check

To be successful in this you may have:

At least 1 year experience in a related area and Certificate in Ageing support, Disability or other related disciplines.

Individuals of Aboriginal and Torres Strait Islander background are encouraged to apply.
