Leading and developing initiatives that positively impact the standards of care within the Palliative Care Sector

Full-time ongoing role available due to internal promotion within a cohesive and collaborative team

The Opportunity

Due to an internal promotion, we have an opportunity to join our Digital Health and Connected Care team, as we continue to work closely with primary care providers and community to deliver sustainable change initiatives in the Palliative Care sector.

Your new role will be to use your exceptional project planning and stakeholder engagement skills to develop and implement key palliative care initiatives across the Murray PHN catchment, whilst engaging with our Victorian PHN partners to identify and implement state-wide initiatives.

Key Accountabilities

Plan, oversee and lead the ‘Greater Choice at Home for Palliative Care’ program through to completion.

Develop and deliver project plans ensuring inclusion of emerging approaches in palliative care align with Murray PHN Strategic and Activity Work Plans.

Engage with a range of internal and external stakeholders to support the development of new models of care and the embedding of existing models.

Manage risk to completion of the project and ensure transition to business as usual is sustainable.

Consult with relevant internal and external teams to develop and align relevant guidelines, tools and reporting that facilitates system-wide change.

Lead employee development and foster team cohesion, providing feedback and coaching as you work collaboratively to improve access to safe, quality palliative care.

About You

We are seeking a program or project leader with substantial experience in primary health and an understanding of acute and subacute services.

Key Capabilities

A tertiary level qualification in health, social science or related field of study plus a minimum five years’ relevant experience.

Experience in project management combined with the ability to apply the principles of change management.

Strong relationship building skills demonstrating adaptive communication and the ability to influence a diverse range of stakeholders.

Understanding of at home palliative care including Residential Aged Care Facilities.

About Murray Primary Health Network

Albury, NSW, Australia

Murray PHN (Primary Health Network) commenced on 1 July 2015 as a part of the national network of 31 PHNs across Australia, which replaced the Medicare Local program.

Murray PHN works closely with the health system to identify areas to improve, either through the better coordination and support of health services or by commissioning new services to address needs.

The catchment of Murray PHN is significant in both size and diversity. Spanning northern Victoria, and including Albury, NSW, the Murray PHN covers an area of almost 100,000 square km and it will impact health services that are accessed by more than 563,000 people.

The Australian Government has identified national objectives and priority areas for PHNs. These frame the focus of Murray PHN and will be guided by both community and clinical input to improve health outcomes.
