About the Role
You will work within a flexible, multidisciplinary structure, that ensures the delivery of high-quality assessment and management services to Memory and Geriatric clients, making sure that the client's and families' needs are met in a timely manner. The role is currently based at St George's Hospital, Kew. You will conduct triage, comprehensive pre-clinic assessments, develop the client's care plan and provide ongoing education and service linkage for clients and families as appropriate.

You will be the patient and family's contact person throughout their journey

Classifications for this position will range from classification range VG3 - VG6 ($104,135.20 - $118,981.20 per annum).

Your Contribution

• You will have a Bachelor of Allied Health discipline (eg Social Work, Occupational Therapist, Speech Pathologist) or equivalent or Bachelor of Nursing with minimum of 7 years of Allied Health, Aged Care or Nursing experience with dementia and/or psychiatric illness in aged clinical populations in a community based, multidisciplinary setting.
  
• Extensive knowledge and experience of dementia syndromes and psychiatric illness in aged clinical population and their management.
  
• Detailed knowledge of dementia and aged care service systems and community supports for clients/carers.
  
• Demonstrated ability to work in multidisciplinary teams, at an advanced level.
  
• High level of understanding of the impact of a diagnosis of dementia on the individual and their families
  
• Highly developed assessment skills Ability to provide post diagnosis education, counselling and future care planning to families/ carers.
  
• Highly developed individual, family work and crisis intervention skills
  
• Highly developed interpersonal skills. These should incorporate qualities of excellent communication (verbal and written) and skills in collaboration, negotiation and conflict management
  
• Current National criminal history check, or willing to obtain

About CDAMS and Geriatric Medicine Clinics - Health Independence Program (HIP)
The CDAMS & Geriatric Medicine Clinics are part of the HIP Specialist Clinics. Services may be provided in the client's home or in the centre. The role involves interaction with GPs, medical specialists, client's, family and carers, other SVHM and HIP services.
